    I was told by our Reservations manager that she left you a voicemail and that your medical paperwork was faxed to you.

    Please let me know once you have received you paperwork.

    Yes; received at 10AM EST on 2/18/2013, received phone call shortly thereafter to confirm FAX received. FYI, never did get email that included forms but that is OK now that they arrived in a FAX. Thank you
